چکیده
Objectives: Our objective was to study the potential utility of multidetector computed tomography (MDCT) to identify both cardiac embolic sources and coronary artery disease (CAD) in embolic-stroke patients. Methods: We performed MDCT for 184 patients with embolic stroke but without known CAD. Twenty-six patients had atrial fibrillation. We investigated the prevalence of the potential source of the embolism and the coronary characteristics. Results: Overall, 64 potential embolic sources were detected in 59 patients (32.1%). Left atrial appendage thrombus, left ventricular thrombus and aortic atheroma were detected in 3.3, 0.5 and 15.8% of patients, respectively. Circulatory stasis and patent foramen ovale were detected in 8.7 and 6.5%, respectively. As for coronary calcium score, only 47 patients (25.5%) had a score of zero and 51 (27.7%) had a score of ≥ 400. Significant CAD was detected in 18 patients (9.8%). One hundred and thirty-seven (74.5%) had coronary plaques. The prevalence of positive remodeling, low-attenuation plaque, spotReceived: June 19, 2015 Accepted after revision: September 23, 2015 Published online: December 1, 2015 Kohichiro Iwasaki, MD Department of Cardiology, Okayama Kyokuto Hospital 567-1, Kurata Naka-ku, Okayama (Japan) E-Mail kiwasaki @ kyokuto.or.jp © 2015 S. متن کاملANGIOPLASTY AND STENTING OF CAROTID ARTERY STENOSIS WITH EMBOLIC PROTECTION DEVICES
Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...
متن کاملBilateral embolic protection devices for high-risk cardiac surgery in a patient with recent embolic stroke.
C erebral embolic protection devices (EPDs) such as carotid filters were first introduced as an adjunct to carotid artery stenting out of concern for increased rates of procedure-related atheromatous plaque embolization compared with carotid endarterectomy. Here, we report the use of a carotid filter device for embolic protection during high-risk coronary artery bypass grafting and ventricular ...
متن کاملLacunar infarction as an embolic complication of cardiac and arch angiography.
BACKGROUND Lacunar infarcts are small, deep cerebral infarcts resulting from occlusion of small, penetrating cerebral arteries. They are most commonly associated with hypertension. Cardiac sources of embolization are sometimes present in patients with lacunes, but direct proof of a causal relationship is lacking. This report attempts to support the contention that emboli can cause lacunar syndr...
متن کاملVitamin E Derivative Alpha-Tocotrienol Failed to Show Neuroprotective Effects after Embolic Stroke in Rats
Objective(s) Previous studies have demonstrated that pretreatment with alpha-tocotrienol (a-TCT) can reduce ischemic damage in mice following middle cerebral artery (MCA) occlusion. It is also reported to decrease stroke- dependent brain tissue damage in 12-Lox-deficient mice and spontaneously hypertensive rats. In the present study, the neuroprotective effects of a-TCT and rosiglitazone (RGZ)...
